As most you are aware the State Bureau of Vital Records has acquired a new application of vital events. The anticipation is that this will make Arizona a 100% electronic state. Something that is changing is that with D.A.V.E. medical certifies will be added in this electronic system in addition to Funeral homes and others who already have access to VSIMS. So these medical certifiers will be given an I.D. and password. All physicians will be required to certify death certificates electronically with this new system. Arizona has adopted a work flow that requires complete registration of the record before a funeral home will be able to print a disposition transit permit.

How do I get into DAVE to fill out a death certificate. The link I was sent appears to be for training only, which has been completed.The password and username (my email) does not work on the main DAVE site. It is working only the training site.
